IP01 - Syndromic vs. Non-Syndromic Vascular Anomalies: A Decade of Outcomes from a Mexican Referral Center

Objectives: To describe the clinical spectrum, anatomical distribution, treatments, and outcomes of vascular anomalies managed in a national referral vascular surgery center in Mexico, comparing syndromic versus non-syndromic presentations, and identifying gaps in ISSVA-aligned and genetics-informed care.

Methods: A retrospective cohort (2014–2024) included all patients with vascular anomalies treated in a tertiary vascular surgery service. Fifty-six patients were analyzed after excluding 2 with non-vascular neoplasia. Variables included demographics, ISSVA classification, anatomy, symptoms, treatment modality, clinical response, adverse events, and genetic testing. Groups were compared using t-tests and Chi-square/Fisher tests (two-tailed p< 0.05).

Results: Table I. Non-syndromic anomalies predominated (51/56; 91.1%), while 5 patients (8.9%) had syndromic disease (p < 0.0001). Age was similar (25.4±17.2 vs. 25.0±19.2 years; p=0.960), with overall female predominance (57.1%; p=0.379). Head and neck were the most common sites (46.4%), followed by lower extremities (33.9%), the latter significantly higher in syndromic patients (80.0% vs. 29.4%; p=0.041).

Pain (67.9%), edema (58.9%), and cosmetic changes (48.2%) were the main symptoms. Embolization was the principal intervention (85.7%), followed by surgery (46.4%) and sclerotherapy (33.9%), with syndromic patients more frequently undergoing sclerotherapy (p=0.041). Clinical improvement occurred in 92.9% (p=0.32).

Adverse events occurred in 32.1% of patients; syndromic patients had significantly fewer event-free courses (p=0.033) and more atypical complications (p=0.019). One Klippel–Trénaunay patient required lower-limb amputation after repeated embolization. Only one patient (1.8%) underwent genetic testing.

Conclusions: Non-syndromic anomalies predominated, but syndromic disease showed distinct anatomical predilections and a significantly higher complication burden. Despite similar rates of clinical improvement, syndromic patients required more multimodal therapy and experienced more adverse events. The extremely low rate of genetic testing highlights a major gap relative to ISSVA recommendations. Findings support structured pathways incorporating systematic ISSVA-based stratification, early genetic evaluation, and tailored multimodal management for complex vascular anomalies.
