Placement and management of arteriovenous fistulas and grafts in complex hemodialysis patients can present significant clinical challenges. We will review strategies for access placement in elderly, pediatric and obese patients and patients with breast cancer and implantable cardiac devices and discuss management of complications including recurrent access thrombosis and hypercoagulable states, ischemic complications, and infections.
