Summary: We present a short video of a case demonstrating ex vivo renal artery aneurysmectomy with kidney autotransplantation for management of a large, complex renal artery aneurysm complicated by prior endovascular coiling. The key steps of this operation include nephrectomy, back-table vascular reconstruction, and autotransplantation. This highlights a potential surgical approach to preserve the kidney in scenarios where in situ or endovascular repair is not feasible.
