Summary: Laparoscopic entry into the retroperitoneal space enables direct access to lumbar arteries. Through this approach, we were able to access the posterior aspect of the aneurysm sac from the left renal artery down to the iliac bifurcation. Laparoscopic lumbar artery ligation can be considered for the management of persistent type 2 endoleaks in patients who are high-risk for open conversion or have failed coil embolization.
